AWARD PROCESS RULES . Entries must have been active online during 1999 All entries must provide the web URL of the specific page of entry. (Entries may be other than "main" pages.) All entries must provide a printed sample of the specific page of entry. (Color is recommended but not required for the print sample.) Each entry must be submitted separately. Each entry must be accompanied by TWO (2) entry forms:...One PRINTED FORM, clipped to a printed sample of the entry ...One ELECTRONIC form submitted online or via email. Submissions MUST arrive on or BEFORE October 15, 1999. Entry materials will not be returned. All entries selected for showing will be contacted via email. All Judges and ballet decisions are final. Judges reserve the right to reassign categories if necessary. . USE THE FORM PROVIDED Once you have completed the online entry form, save the form as TEXT and print copies to attach to the entries.
